NAME, SOCIAL SECURITY NUMBER, DATE OF BIRTH, ADDRESS, SEX, HOME PHONE NUMBER, EMAIL ADDRESS, ARE YOU A US CITIZEN, US NATIONAL OR LEGAL RESIDENT, CURRENT SCHOOL AND LOCATION, CURRENT GPA, CURRENT MAJOR, ARE YOU CURRENTLY EMPLOYED, AMOUNT SAVED FOR COLLEGE, WRITE A 350 TO 500 WORD ESSAY THAT DESCRIBES THE APPLICANT'S EDUCATIONAL AND PROFESSIONAL GOALS, AND WHY THEY NEED THE SCHOLARSHIP. TWO LETTERS OF RECOMMENDATION (ONE MUST BE FROM A CURRENT HIGH SCHOOL TEACHER OR UNIVERSITY PROFESSOR, BUT NEITHER CAN BE FROM A RELATIVE), AND A COPY OF HIGH SCHOOL TRANSCRIPT (AND COLLEGE OR UNIVERSITY, IF APPLICABLE), AND any standardized test scores related to the plannededucational pursuits (EG ACT, SAT, LSAT, GRE, GMAT).

- The applicant must maintain a GPA of 3.0 or better and must take at least 12 hours to maintain their scholarship.
