If the foundation determines the letter of inquiry falls within its interest areas the applicant will be notified by mail to submit a full proposal. The full proposal must include description of project, description and history of the organization, project staff background and expertised, amount of request, need for project, and support from other foundations and/or corporations. The following information must also be included: financial statements (two years), form 990 (two years), budget for current fiscal year, project budget, board list and IRS determination letter.

THE FULL PROPOSAL MUST INCLUDE DESCRIPTION OF PROJECT, DESCRIPTION AND HISTORY OF THE ORGANIZATION, PROJECT STAFF BACKGROUND AND EXPERTISED, AMOUNT OF REQUEST, NEED FOR PROJECT, AND SUPPORT FROM OTHER FOUNDATIONS AND/OR CORPORATIONS. THE FOLLOWING INFORMATION MUST ALSO BE INCLUDED: FINANCIAL STATEMENTS (TWO YEARS), FORM 990 (TWO YEARS), BUDGET FOR CURRENT FISCAL YEAR, PROJECT BUDGET, BOARD LIST AND IRS DETERMINATION LETTER.
