Competitive funding for national or multi-site initiatives that improve care for older adults and/or support family caregivers, align with at least one of JAHF's three priority areas, and use evidence-informed, scalable and sustainable models of care or policy implementation. Up to 10 grants will provide up to $300,000 each over two years ($150,000 per year). Eligible applicants: For nonprofit organizations, the organization must not have received a board-authorized JAHF grant in the last 30 years; priority will be given to organizations that have never received a board-authorized JAHF grant under its Aging and Health program.; For academic institutions, the applying department or division must not have received a board-authorized JAHF grant in the last 30 years; priority will be given to organizations that have never received a board-authorized JAHF grant under its Aging and Health program.; Recipients of limited or one-time discretionary awards remain eligible.; Eligible applicants must be U.S.-based 501(c)(3) public charities, colleges or universities, hospitals, or health care organizations conducting work in the United States.; Proposed work must primarily benefit older adults and/or family caregivers in the United States and have a national or multi-site scope.; Applicants must comply with applicable federal restrictions on lobbying.; An organization may submit only one proposal, except that separate departments or divisions may submit separate LOIs when they address distinct topics with non-overlapping implementation sites.; Proposals must align with at least one priority area: Age-Friendly Health Systems, Family Caregiving, or Serious Illness and End-of-Life Care.; Successful proposals must include evidence-informed models of care or policy implementation activities that are scalable and sustainable.; Projects focused on a few sites within a limited geographic area will not be competitive; multiple sites within one state may be considered if the initiative is scalable nationally.; The number of implementation sites must be adequate to demonstrate scalability at a national level.; All else being equal, projects focused on equity and underserved or marginalized populations will be more competitive.; Early, discovery-stage research will not be competitive; evaluation or study of implementation activities is acceptable.; Single-site projects are not supported.; General operating support and capital projects, including construction or renovation, are not supported.; Programs principally delivering direct services, such as routine clinical care, legal representation, transportation or meals, are not supported.; Films or documentaries, books, and art projects are not supported.; Competitive applications must either include partnerships or explain why a single organization can accomplish the stated goals.; An eligible new partner may include an existing JAHF grantee as a subcontractor or collaborator, but the initiative must involve activities different from that grantee's existing JAHF-funded work.; Applicants must first register for the online grants portal before submitting an LOI.; The LOI must include the organization's most recent Form 990 and audited financial statements; applicants lacking these must provide comparable documentation, including other IRS documentation establishing tax-exempt status or eligibility to receive charitable contributions when no Form 990 is available.; Applicants invited to submit a full proposal must provide a detailed project narrative, workplan, budget, budget justification and other required materials through the same portal.; Changes between the LOI and invited full-proposal stages must be limited and consistent with the selected LOI; requested changes require written explanation by email and JAHF approval after review.; Indirect costs may not exceed 12.5% of direct costs, excluding subcontracts; subcontractors may include indirect costs up to 12.5% of their own direct costs if that amount is not also included in the applicant's budget.; Competitive applicants must show that activities will continue beyond the funding period or explain why continuation is unnecessary to achieve the proposed scalable outcomes, including plans as applicable to sustain work, disseminate findings, support adoption or secure future funding.; No more than 50% of an individual's salary and benefits may be charged to the grant; effort beyond that level must be supported by other funding sources..
