Applications are submitted by invitation only in written format and include: 1) a project description describing the proposed project. 2) a brief history of the organization's accomplishments and mission, and qualifications of individuals involved. 3) a concise description of the methods used to evaluate the results of the proposed project. 4) a list of the officers and directors of the organization and the percentage they contribute to the overall income of the organization. 5) a list of full-time and part-time staff positions and a short description of each job function. 6) a detailed budget of the project and the annual operating budget of the sponsoring organization. 7) a donor report showing a list of major contributors and approximate levels of giving. 8) a list of additional fund sources sought for the project. 9) an IRS determination letter verifying the tax-exempt status of the organization.
