Eligible applicants: Only organizations that are tax-exempt under IRS Code Section 501(c)(3) are eligible for grant funding.; The program must align with at least one of the Foundation's focus areas: basic needs, education, mental health, or positive youth development.; Requests are not considered for endowment funds.; Requests are not considered for mass funding appeals.; Requests are not considered for sponsorships.; Requests are not considered for advertising for fundraising event tickets.; Requests are not considered for grants or loans to individuals.; Requests are not considered for political campaigns or activities.; Requests are not considered from organizations outside the United States.; The Foundation accepts one application per year from any qualifying organization, defined as four quarters following the date of its last application.; All grant requirements, including a final report, must be completed before the organization can be considered for further funding.; A reasonable request should account for its proportion to the overall program budget and demonstrate support from other funding sources and community supports; ongoing work should also be sustainable.; First-time portal users must create and register an account using the organization's Tax ID/EIN.; Applicants directed to the Grant Interest Form must complete it as the first stage of a two-stage application process and must be invited before a Full Application becomes available.; Applicants directed to the Returning Application use a single-stage process and do not have to complete a Grant Interest Form.; The application requires organizational vision and mission statements, history and purpose, and a brief description of services and service area.; The application requires a project or program description, statement of community need with relevant and preferably local supporting statistics, population profile, amount requested, collaborating partners and their roles, pending and committed funding from other funders, and a sustainability plan beyond the requested funding period.; The application requires an evaluation plan identifying tools or methods for measuring progress and success, an explanation of how the organization defines project success, and an explanation of how evaluation results will be used.; Required supporting documents are an IRS determination letter verifying tax-exempt status, Board of Trustees list, project or program line-item budget, current-year organizational budget, preceding-year organizational budget versus actual results, most recently filed Form 990, and most recent audited financial statements.; After submission, applicants might be asked to provide an annual report, strategic plan, Form 1023, and current financial statements including a balance sheet, income statement, and statement of cash flows.. Geography served: United States; Montgomery County, Ohio; Greater Dayton area. Published application deadlines: FULL APPLICATION SUBMISSION DEADLINES: February 1 May 1 August 1 November 1.
