Emergency Need Grant

Open · closing date not stated

Grants to 501(c)3 organizations that provide emergency financial relief to patients with solid tumor or hematologic malignancies in Northern New England who face financial toxicity or food insecurity (for example, costs of non-covered medical supplies). Funds go to the organization, not to individual patients. Awards are up to $3,000 per site per year across both Cancer Community Support programs, with an average per-patient target of $375 and at least six patients helped per grant. Eligible applicants: Only 501(c)3 organizations may apply.; The submitting organization must have existing infrastructure (staff, screening processes and tracking systems already in place, not planned) to identify eligible patients, disburse funds and report on use.; Grant funds go to the organization, not to individuals.; Patients helped must have a diagnosis of a solid tumor or hematologic malignancy and be receiving active treatment or palliative care as they approach hospice. The program is not intended for patients with benign hematologic conditions or long-term survivors not in active treatment.; Funds shall not apply to any per-patient annual assistance cap.; Organizations choose one program per cycle; the $3,000 annual cap per site applies across the Grocery/Gas Card Program and the Emergency Need Grant.; Organizations may apply once a year. If funds remain in the fourth quarter, past recipients will be invited to reapply.; To be eligible, the previous award must be fully expended and all required reporting submitted and accepted by NNECOS.; Applications describing per-patient awards substantially above the $375 average target, or whose main activity is redistributing funds to other organizations, are outside the program's scope.; Strong applications describe a specific patient population with realistic volume, name the staff responsible for identifying patients and disbursing funds, and give a per-patient award estimate consistent with the program model.; Reviewers look for geographic alignment with Northern New England and evidence that NNECOS funds will complement, not replace, other assistance resources already in use.; Applications are evaluated against a program rubric.; Reporting is required at regular intervals under the terms and conditions signed before the award.. Geography served: Maine; New Hampshire; Vermont.

Grocery/Gas Card Program

Open · closing date not stated

In-kind support

In-kind support for organizations. NNECOS buys grocery and gas cards and provides them in branded envelopes to participating practices, clinics and 501(c)3 organizations, which distribute them to patients with solid tumor or hematologic malignancies who face food insecurity or transportation burdens during active treatment. Each site may receive up to 120 cards a year ($25 each). Eligible applicants: Open to practices, clinics and 501(c)3 organizations.; Organizations may take part in only one program (Grocery/Gas Card Program or Emergency Need Grant) at a time; a $3,000 annual cap per site applies across both programs.; Patients receiving cards must have a diagnosis of a solid tumor or hematologic malignancy and be receiving active treatment or palliative care as they approach hospice. The program is not intended for patients with benign hematologic conditions or long-term survivors not in active treatment.; A champion at the site must take responsibility for card distribution, tracking and accountability reporting to NNECOS.; Documented accountability procedures/protocols are required.; All sites may apply once per calendar year.; To be eligible, the site's previous award must be fully expended and all required reporting submitted and accepted by NNECOS; a site may not return for more funding until both conditions are met.; Strong applications describe an existing, functioning patient assistance infrastructure rather than a plan to build one: a named champion with a defined role, a clear process for identifying eligible patients, realistic estimates of patients served, and capacity to distribute cards steadily. Applications that cannot describe how patients will be identified and how distribution will be tracked are unlikely to be funded.; Applications are evaluated against a program rubric.; Site champions must report at regular intervals under the terms and conditions signed before the award.. Geography served: Maine; New Hampshire; Vermont.
